Once your home institution has nominated you to participate in Erasmus mobility, you should complete the Erasmus application, which consists of the following:

1. Application Form
2. Learning Agreement for studies or Learning Agreement for traineeships; approved by your departmental coordinator
3. Transcript of Records from your home University (signed and stamped by responsible person)
4. Copy of valid Passport / ID card
5. CV in Europass format (in English)
6. Confirmation of your current language level – this confirmation can be issued by your home university or it can be language school certificate (Please note that recommended level is B2)
7. Accommodation request form (if you wish to be placed in a students dormitory) – University of Applied Health Sciences does not guarantee placement, it depends on the availability of rooms in the dormitory

Application form, Learning Agreement, Transcript of Records and Confirmation of Erasmus student status  need to be duly signed and stamped by your home university. Please note that we can only deal with applications that are duly completed and submitted with all the documents required on or before the relevant deadline. If any documents are missing, we will advise you by e-mail, but your application can only be processed once we have received all the documentation. Late applications will not be considered. Once you have all the documents, a scanned copy of each needs to be sent to the e-mail address: erasmus@zvu.hr. Please send clear scans (not photos) of the documents with clearly visible signatures and stamps.

* Health insurance All exchange students need to have health insurance that is valid for the whole period of mobility. They are required to present an evidence of health insurance prior to their arrival (send a scanned copy ) and upon their arrival as well.

1. Students arriving for the purpose of studies need to have EHIC (European Health Insurance Card) which covers basic costs of health insurance. Erasmus students are advised to have additional insurance coverage against accidents and civil liability (option).
2. Student arriving for the purpose of traineeship need to have (in addition to general health insurance – EHIC) accident and liability insurance as stated in the LA. Evidence of the insurance needs to be provided prior and upon arrival as well.

 STUDENTS ARE REQUESTED TO PRESENT THE ORIGINAL DOCUMENTS UPON ARRIVAL TO THE UNIVERSITY OF APPLIED HEALTH SCIENCES (TOGETHER WITH 2 PERSONAL PHOTOGRAPHS). After the deadlines the student will receive a Letter of Acceptance issued by the University of Applied Health Sciences.
