Headache-Attributed Restriction, Disability, Social Handicap and Impaired Participation

 

Opis upitnika

U svrhu podizanja svijesti i spoznaje o glavoboljama, prikupljanja epidemioloških podataka, podataka o utjecaju onesposobljenja prouzročenih glavoboljom na pojedinca i društvo te identificiranja zdravstvenih potreba koje će rezultirati prepoznavanjem potrebnih promjena Svjetska zdravstvena organizacija 2011. godine podržala je projekt Globalne kampanje protiv glavobolja, koji provodi neprofitna organizacija Lifting The Burden. Preporuke Globalne kampanje protiv glavobolja precizno upućuju na metodologiju populacijskih istraživanja prevalencije, opterećenja i troškova liječenja glavobolja. Preporuke se odnose i na mjerni instrument: HARDSHIP upitnik (engl. Headache-Attributed Restriction, Disability, Social Handicap and Impaired Participation (HARDSHIP) questionnaire).

HARDSHIP upitnik je strukturirani upitnik u kojemu se uz demografske podatke postavljaju pitanja probira, dijagnostička pitanja temeljena na ICHD 3β kriterijima, pitanja o opterećenju simptomima glavobolje, onesposobljenjima, osobnim financijskim opterećenjima, kumulativnim i sveukupnim opterećenjima, pitanja o korištenju zdravstvenim resursima i posljedičnim socioekonomskim opterećenjima. Preporuka je da se podatci prikupljaju anketom koju provode adekvatno educirani ispitivači, ne nužno zdravstveno osoblje. Dokazana je osjetljivost i pouzdanost HARDSHIP upitnika u različitim kulturama i govornim područjima te se preporučuje primjena upitnika u populacijskim istraživanjima o prevalenciji i opterećenjima glavobolje.

Abstract

Aims: As a prerequisite for a cross-sectional study of the prevalence and burden of primary headache disorders among University of Applied Health Sciences students in Zagreb, Croatia, we undertook cultural adaptation of the Headache-Attributed Restriction, Disability, Social Handicap and Impaired Participation (HARDSHIP) questionnaire and its translation into Croatian language, validating the headache screening and diagnostic questions.

Methods: In two stages we first adapted and translated the HARDSHIP questionnaire from English into Croatian, then validated the diagnostic module in a patient sample in clinical settings, and in a convenience sample of 150 participants in the main survey, consecutively selected according to their questionnaire-derived diagnoses and invited to re-interview and examination by a headache expert blind to the questionnaire diagnoses. Sensitivity, specificity and positive and negative predictive values were calculated for migraine and tension-type headache, comparing questionnaire-derived and expert (reference) diagnoses.

Results: No major problems were experienced in translation and adaptation. The questions were found culturally inoffensive in Croatian language, and the length of the questionnaire acceptable. For the screening questions, we found 100% agreement in headache caseness versus non-caseness (no headache in the preceding year) in the validation subsample of N=145 (five refused to participate in the validation). For the diagnostic questions, we found very high sensitivity (95%) and high specificity (89%) for migraine, with positive predictive value of 89%, negative predictive value of 95% and κ=0.85 (very good agreement). For tension-type headache we found somewhat lower sensitivity (78%), very high specificity (94%), positive predictive value of 83%, negative predictive value of 93% and κ=0.74 (good agreement).

Conclusion: The Croatian-language version of HARDSHIP is validated and ready to use in epidemiological studies.
